EDIT: After messing with the mission it looks like whenever you initiate his second phase and he teleports to the left side of the room. Is where the encounter glitches and won’t spawn a weak point! So if you can get him to stay on the right side of the encounter all should go well. Please do not fully restart the mission or return to orbit, just wipe and hope RNG is on your side.
