Warlord’s Ruin Bones Triumph Bug
I was doing Warlord’s Ruin to collect all the bones for the “Heed the Whispers, O Vengeance Mine” triumph and I died in the maze between 1st and 2nd. It spawned me at 2nd so I went ahead and grabbed the bone there (triumph bone #5). It didn’t count for my triumph. I loaded in again, ran up to this point and there is nothing but a glow on the bone table. I communed and it still did not fill in the box.
I cannot complete the triumph and therefore I cannot complete this seal. Please advise on what to do. I have a video but I’m not sure how to add it.

I have the excact same problem. I first ran the dungeon with someone who had access to some of the doors. I collected 2 bones before then completing the dungeon for the first time, which messed up my entire quest because I didnt get the bones in the order Im supposed to, but Im not sure if that matters for the issue of me being unable to cellect the bone because all the other ones still counted towards my triumph progress anyway. Now I got all of the bones but im missing the Triumph bone #5. I was sure i already got it so i went back and tried to get it but it was gone, only the option to commune was there which gave me some dialogue from crow, but not the bone in the triumph. I went back and tried to get it multiple times on different runs, and even tried different characters. Despite all this, it never worked, so Im unable to complete the quest and the seal, and of course im also missing out on the boost rate to buried bloodline. Im sure its not a coincidence that its the same bone in both our scenarios.
