The Tormentor spawned in Vostok and got pushed back into the cave section. After a few seconds he did the animation like he was being removed from the match, but ended up getting put inside the inner cave circle walls. He was then stuck there with no way to kill him and end the round. Match got completely bricked at wave 39. He was moving around inside that area and kept running back and forth down there as you can see in the screenshot.
