You are now a multi-millionare, aside from paying off taxes, bills, loans, etc. what's the first thing you do?

Buy my mum a house. Load up the bank accounts of close family and friends. Buy several motorcycles and fund various car and motorcycle projects, starting with a 13B swapped Chevy C10 in memory of my dad. Guitars, many, many guitars. Finish ruining my body with more tattoos. Invest and help those in need.
