I think the easy fix is to "change character" when you are in the transition to entering the match with your teammates ships. On the PC it's the escape button for the list that has 'change character' on it. Me personally, once the match has registered as over I immediately go to the map and select either another match or go to HELM or tower or any other place.

I don't know if console is the same but on PC you can go to settings, scroll down to gameplay, and change characters there. It's been doing the same to me for the past week or so. Upon exiting any crucible activity it hangs right about where it should be matchmaking. I can get into my menu system, but it won't let me exit, change character, or anything else as you normally would. SO it was pointed out last night to me that you can simply change characters through your settings menu, in gameplay. Hope that helps.

This happened to me four or five times yesterday, and I've been doing the same thing you've been doing - back out of the game and relaunch. I'm on Steam. Crossing my fingers this won't happen as often this weekend lol
