yep came here to post this exact thing. I've been running solo Duality a ton since it came out and never saw it until my run last night. Happened twice in one run and the first one killed my solo flawless attempt at the vault. Someone in another post said they think it's a collision issue with an enemy in the nightmare world but I disagree, I never saw it until this week and the second time it happened I was midair jumping up to the psion platform as I teleported - absolutely nothing I could have collided with. Definitely think it's a new bug since the patch last reset too.

I have over 90 clears. Since season of the haunted it’s gotten 10x worse. More so on the 2nd and last encounters. Standing in different places near or far to the bell or moving when outside. Then standing/moving right next or touching the bell inside the nightmare realm. It also appears to be bugged if the bell is being shot from multiple guardians (single bell) and or multiple different bells. Also if being shot from a weapon that can close it in one shot then shot again. All of these examples can insta kill 1-2 or wipe all. There is also a bug that teleports 1-2 or every one to the rally flag in the last encounter when the bell is rung. This really needs to be fix asap
