Ascendancy Ornament bug

I collected the ornament for "Ascendancy" from Shaxx, "Final Ornament" but it isn't showing up on my collections nor on modifications.

Dumb question, is it in your character inventory screen, where as you have to unlock it first, like some armor ornaments?

but you can equip it, right? If so, it is the same as with "No Time To Explain", that ornament was also shown in the collections as not having gotten
