Xenophage Quest Lost Sector Puzzle K1 Crew Quarters Not Working
Have the correct combination and nothing is working for solving the puzzle. Tried reloading the lost sector twice and still nothing

Same here. Was helping a friend get it and it we tried it twice in the Crew Quarters and it does not work using the: K1 Crew Quarters: 3 - 4 - 5 - 8 - 9 - 9 - 8 - 8 Combination (9 symbols are designated as top left = 1, bottom right = 9)

I'm having the same issue. Also, if you put it in wrong how long do you have to wait to try again?

If you are using a guide for solving the puzzles, you may want to check other guides than the one you are currently using. Not all guides are accurate, it would seem.
