I have the same issue and have been having it since roughly the start of Forsaken. Still continues today after the Opulence patch.
Intel Core i5 6600k @ 3.5GHz
Nvidia GTX 980 Ti
16GB RAM
Windows 10
144Hz monitor.
I've tried all manner of vsync and frame cap "fixes". Nothing improves it :(
Seems to be at it's worst in open world areas and during public events
