PC crashing so often I can't finish Forsaken campaign.

System info inside Spoiler tag
BattleTag JU1CEB0X#11241 Configuration Info CPU: Intel i5 - 4690K @ 3.5Ghz GPU: EVGA NVIDIA GTX 970 RAM: 16 GB GPU Driver Version: 24.21.14.1163 (released 9/18/18) Operating System: Windows 10 Pro (Version 1803 Build 17134.285 DxDiag: • To find this information, press (Windows Key + R) to bring up the Run dialog box, enter this command “dxdiag”, no quotation marks. • Click on “save all information”. • Copy text from saved file to preferred third-party text dump website (i.e. PasteBin, DumpTxt) and share link here: https://pastebin.com/aYBZtR3f
Destiny 2 Forsaken has become unplayable on my system. It has gotten so bad, that I am unable to finish the story on my second character because the mission to unlock the Dreaming City causes my entire system to hard-crash every time I fight the taken (after seeing the 3 techyun (sp) the first time). I have had this issue intermittently since D2 launched last year on PC, but it has become so bad I can't play for more than 10 minutes without my entire system crashing. The sound messes up for a second, monitors go blank (then display the auto-sleep message), audio returns to normal (with game sound), but I hear the Windows chime that something has been disconnected, and I am unable to do ANYTHING. I can't alt-tab, I can't ctrl-alt-del, nothing... I am running dual 25" 21:9 monitors, and both go blank... and I am unable to get any response from my computer. I have to hold the power button for 10 seconds to hard-restart my machine. Upon rebooting, I do not receive any sort of error message except the default Windows message in the Notifications section "Display Driver failed to start; using Microsoft Basic Display Driver instead. Check Windows updater for a newer display driver." I have tried every possible step that I know how to do, from closing all applications (and ensuring they are truly closed in Task Manager), I have reset my CSVARS twice, I tried to repair D2, I tried to uninstall and reinstall D2. I tried using the Windows repair utility to fully reinstall Windows 10. I have read every tip/trick/idea that I can find, but nothing seems to help the crashes at all. I do not have EVGA Precision X installed on my computer, and have not changed any clock-speeds at all from factory-defaults. I have run FurMark overnight (at max settings) to see if I can get my video card to crash, but it runs a rock-steady 21 FPS at 70C all night. I have run 3DMark to see if I can get any sort of glitch in my hardware outside of D2, but nothing else causes any sort of instability. My next step will be to run memtest to verify that my RAM is working... but from what I have read online, it's a Destiny problem, and not a hardware issue inside my machine. Please help!!!
