Once you kill the Mindbender and you are about to leave the Ascendant plane, there are platforms that appear to the exit gate and you jump on them to get a charge. Once you have the charge leave the plane and to your right is a sealed door, deposit the charge and it opens to a region chest with an emblem. As most people would avoid this little tidbit, completionists have a triumph. EDIT: Yes, I know this is old news. But its nice to refresh people who surf forums instead of Youtube. There is also another triumph with the Rifleman where you shoot his holograms 5 times in one mag, I did not get this on my first character (how did i not).
