There was one on Io. It was sitting in one of the Warmind vault tunnels on the surface with taken. You walk in, and the room has a door leading out to our left. To your immediate right of the room is a small hole in the wall, crouch through that and the scannable is in the back of the hallway.
I will try to figure out exactly where. I just know what it looks like

In the first one. You can only scan them once each.

You can scan it once on every Rally. If you talk about the Excavation Site II, what lead from Giant's Scar then that is empty now. There was it in previous rally, yes. At the moment it seems Io and Mercury bugged and there is no scannable spawn. We have found 3 so far (EDZ, Titan, Nessus).
