I don't know if these are random, or if they change each day, or each rally, or what, but in the past, I've found a couple:
On Titan, in Siren's Watch, Methane Flush Lost Sector. The boss is a wizard that will move further back into the area as you hurt it. In the final room, in a corner under a catwalk, there was a console of some sort to scan.
On Nessus, Exodus Black, The Rift Lost Sector. The chest is up by a pile of what look like shipping containers. Inside one was some sort of device (can't remember what it looked like) to scan.

Some said here if u pledge on different Faction on your characters then u can scan them on every char. Today I will test it on same Faction. I havent found on Titan yet. In last faction it was there where you mentioned. I have seen a youtube video once about one scannable is not in a Lost Sector on titan but in the Achology zone hidden, up on a platform. You know the zone where the Sirens Song and Deathless Adventure lead you. Edit: crazymike made a tube video about it, scroll down.
