Same issue with me. I bought the physical limited edition from eb games(Canada). Entered the code inside the CD case.
I was able to get the coldheart, killtracker ghost and emote upon finishing the campaign and reaching lvl 20. However 2 weeks ago, the killtracker ghost and emote are greyed out and says I "requires pre order to use item".
On day one I gameshared with my friend's account, but I picked up my own physcial copy on day 2 and have been playing that since. I'm thinking that might be the issue? The game still thinks I'm game sharing off my friend's account, but I have purchased my own copy of Destiny 2, and those two items are locked out.

Although I bought and redeemed on my account on my PS4 (and initially played on my own stuff) there are 3 PS4s in my House. Both of my roommates also have one and also have their own copies of the game. I’m thinking that maybe when I played on my account through one of their PlayStations when the WiFi wasn’t reaching my room very well for a few days that something caused the issue. My emote and cold hart are both fine though
