Have you tried logging out of the game via Switch Profile recently?
Load up a character, hit menu and go over to Settings. Then hover over Log Out and pick the Switch Profile option. You should get put back to the title screen and a dashboard screen to pick a gamertag account. Sign in and then load up a character and see if the achievement pops.
