Lag my friend. It's happened to all of us at some point sometimes 10-20 times a day depending on play time per day. Lag in this game is worse than anything I've ever seen. I've seen people tank rockets, shoot shotgun blasts out of their ass, shoot a super then go into the animation 3 - 4 seconds later. Anything you can think of has happened because of people with shitty internet connections that don't really give a damn about ruining the game experience for other folks.
