It's a bug. Kind of. The perk increases the magazine by a percentage and because the gun's magazine is already small, the increase isn't applying properly. Your shotgun with Extended Mag selected is capable of holding something like 6.4 rounds and it's rounding down to 6 instead of up to 7.

It's not even 6, it's at 5 which is the what it comes with without attachments.

The perk increases the magazine by a percentage and because the gun's magazine is small, the increase isn't applying properly. It's a known bug as far as shotguns are concerned.
