How does one get such avatar as Lady Ghost, The Cellar Door and Spookysteel?

This brings back memories. I remember when linc went to #destiny and told them in order to get his emblem you had to kill 1 Million knights or whatever the fallen guys were called.

Lady Ghost's isn't a solid square emblem. Hers has the Bungie septagon in it.
