Get 50 pulse rifle kills, 3 public events, nightfall, matches of heroic strikes or crucible. Attune the item with your fireteam Collect SIVA samples from splicers in plague lands, complete 3 matches of archons forge, kill sepiks perfected. Attune the item with fireteam Complete the raid. (This is how far I am, I know afterwards you have to attune the item again, but not what comes after)

The item itself can be inspected, you will see various slots that require some math to work out and match between the fireteam.
