I see you have 9 deleted characters on your playstation account.
Is it possible that one of those deleted characters are the one that obtained the exotic sword(s)?
If so... that's the issue.
You will have to obtain the quest again on one of your current characters.

I have deleted characters, but I have 2 active characters that do not have the quest available in the abandoned quests kiosk. I know for certain I completed it on my Warlock which is the only character I have not deleted (Day 1 character).

The very first time I completed the sword quest was on my Titan that I deleted, but I've since completed the same exotic sword quest(s) on my Warlock and Hunter.

Did you maybe forget which toon got which sword?

I honestly don't know how the MoT book will handle that. Apparently it wants your Titan back. ;) In a perfect world, it wouldn't matter which character finished the quest, but Destiny is not perfect.
