The transfer process is automatic for your characters and their items. However, the game itself and the DLCs transfer under different conditions. I've linked a page that explains this in detail, but for your convenience I've also posted the reasoning below. • The game itself only upgrades digitally if you bought it digitally on or before January 15th • The Dark Below and House of Wolves will transfer, regardless • The Taken King does not transfer under any circumstances since it's not part of the Year One upgrade programme

All your progress, characters and items are saved to your PSN ID and will transfer as long as you log in using the same account. As for your game licenses, currently TDB and HoW are eligible for a free upgrade to next generation consoles. TTK is not eligible for this and will need to be repurchased. https://www.bungie.net/en/Help/Article/12122 The base game of Destiny will only upgrade if it was purchased digitally prior to 1am PST January 15, 2015.
