Stuck in Vault Xbox One

Can anyone tell me why I keep getting stuck in my vault unable to exit without turning off the game? I'm on the Xbox one. Trying to avoid this bug but it keeps happening. Thanks

I pressed the button next to the xbox button on my controller that looks like 2 squares .
