Actually it's both. Mostly Toland though. Osiris was obsessed with Vex and Dredgen Yor went to the moon so it is safe to assume he was driven insane through the Hive.

Not sure. There is a grimore card which contains a conversation between Toland and Eriana-3 where Toland seems to be very close to knowing the song so I would guess during or sometime after
