The iPhone app stole my Black Hammer!!

I changed character but forgot to get my Black Hammer out of the Vault so I used the Destiny Companion app and now I have no Black Hammer anywhere. It's completely disappeared. Is this a known glitch?

Restart console and phone Phone issue disappears sometimes rsndomly but its still there
