Especially with a titan it really isn't very hard. I jump to the first and then second disappearing platform, and then (with the increased height jump) jump off towards the ledge. then let up when I know I'm far enough to land on the ledge, and then hit the jump but again right before I land so I don't die from fall damage. Then you just have to make sure you jump over that little crack.
