You have to get like 5000 pvp and pvp kills with each class, then there's grimoire for a number of kills/wins in each crucible playlist, pve activity (even patrol VIPs), each enemy type and weapon class.

You my good sir need Cabal-Siege Dancers. I don't think there is a Cabal Ghost 3. Feel free to use my somewhat decent PS4 Grimore as reference. Hope this helps :)
