I think a better concept would be giving players more customization options as they unlock them. Like not being able to add a scar on your character until you go through a certain mission. (But perhaps earning the option to use the options they gained when making another character or general customization?)

[/quote]Ideally some of the meaningful sidequests would enable only a few to actually complete them. Not luck based (like monster drops), but like they require group effort. Or just... difficult things. Real rewards for real challenges. Or actual story impact. I'm a fan of permanent effects.
