working fine for me on Xbox - can do sword heavy + multiple lights and then swap back to primary and throw throwing hammer straight away.
Possibly it could be a bug with the keybinds? my charged and uncharged are on 2 separate buttons

For me its not every time it isn't working. But it's often. I can stand and spam my melee and see the powered melee icon flash red everytime I press it. It's terrible since my main build is strand titan with glaive and sword, so I'll often come flying with my sword, then change to primary to throw a couple of melees to get some extra distance, only for nothing to happen and I just fall into a hole lol. But nice to know that an uncharged melee can get you out of it.
