Insight Terminus strike glitch, fall through map floor and die in tunnel on the way in
Subject says it. If you are on a sparrow you can fall through the floor into space and die, right about at the location where the wait-for-team barrier shows up when applicable. Another silly bug.

Bump. It happened to me last week. I was riding my sparrow into the cave enterance and just road right through the floor, fell through the nothingness under the map, and then died.
