Flawless Pit of Heresy didn't count
I just did a PoH run with an LFG on PC, I joined after they had already started the first encounter but I survived the entire run, we defeated the boss and the triumph even popped on the screen, but it doesn't show as being able to confirm in the triumphs screen.

As others have said, you have to join the fireteam before they load into the dungeon for the flawless triumph. The banner you saw pop up was likely for the stat tracker, not the triumph.

Heyo! The dungeon triumphs (solo, flawless, and solo flawless) only count on full runs, with no checkpoints and no leaving. You'll get credit for a flawless clear, but it won't count for the triumph. Hope this helps.
