I have experienced the same. I reached level 20 and beat the campaign and received the kill tracker ghost but never received the cold heart

Digital or physical copy of the game? If it's digital, then go to your libary and check that all the dlc there is installed, if it's a physical copy then did you input the code found inside the game case to recieve the cold heart?
