Anyone having an issue with this, be sure to check that your sword is fully upgraded. There is an additional upgrade node revealed when you infuse it. That node must be upgraded to activate the Sword Reforged quest. Some of the guides do not mention this.

Not 100% sure but i think this did it for me, so ty! 1st time i'm ever doing this quest chain, got the quest from Shaxx to farm materials on Mars (so i took void). Strangely enough the quest was nowhere to be found :S, I also checked my abandoned quests, nothin there. Then i went to Mars, farmed about 15 relic iron with no special kind of materials, read the comment I'm replying to right now and used some motes of light to upgrade the damn sword. After that I visited the tower and lord Shaxx skipped me (no completion of the "lost" quest) straight to the quest where i'm required to kill guardians and majors with the sword. For any1 missing the quest, try upping the sword with motes of light for instance. Once again my thanks @Dactylo!
