Last time I did it, there were a few. Crota took a swing at me well after I was invisible, and charged someone down all the way to the left when we was supposed to be Crotating right.

We were having a hell of a time with it. Hit him with 3 Gallys and two truths all together 10 or more rockets and he wouldn't go down. He did that a bunch. Couple times each run. As well as the things you mention.
