The most annoying accidental death
I know there's a million ways to accidentally die in this game so I want everyone to post their least favorite death and throw the audience the funny details with it too

In the hive room in PoE, if you stand near the back of the right platform and fire a rocket, you will blow up for unclear reasons despite having what appears to be a clear shot. I think the low ceiling triggers it. Had it happen on round three, leading to a wipe, but I assumed a thrall I didn't see got in the way. Got to round three and it happened again, leading to another wipe :(
