I don't think skin or eye colour should be changeable because it ruins the realism for me. But definitely hair, hair colour and face paint.
Oh and big yes to BEARDS!

Beats having a lvl 20 character with 50 hrs of game time that you do not like the eye/skin colour of but cannot change due to lack of "realism".
