Okay so, in an adventure on Nessus, Failsafe is reminiscent and mourning one of her crew members. Your ghost sympathizes with her and states,
[quote] "Must have been hard to lose them. I don't know what I would do if...Never mind." [/quote]
It's pretty obvious that he's talking about our guardian permanently dying. But for a guardian to perma die, wouldn't their ghost have to be destroyed?
Most of the ghosts we see that are not destroyed and their guardian is dead are deactivated. So how would our ghost be able to do anything about your death?

Well if notice many times throughout the lore Ghosts actually have the ability to change Guardians. Not often but it happens. The real question is why are some able to do this? Pull metal and flesh out of thin air, but can’t patch a bullet hole? I think it has more to do with DNA. Each Ghost is specifically coded for particular DNA strand.

In one of the ghost stories. A guardian had metal issues serious ones at that. One day the guardian died and the ghost said -blam!- it and just dint bring him back.

If a ghost cannot revive their guardian, the bond of light between them fades, until they are no longer 'connected', after that, the ghost will try to find a new guardian

Supposedly one of the drifter lore bits mentions a ghist being unable to revive a guardian who died in the middle of the night, apparently some kind of spooky darkness creature came and drained him of light while he slept. Poor ghost probably felt like a failure.

Memories rusty, but wasn’t Tevis separated from his ghost by a vex gate when he was killed? I don’t remember his ghost being destroyed. But he was dead dead.

Well, if you get separated from your Ghost and you die, he won’t be there to revive you.

Didn't he revive us from basically skeletal remains at the start of D1? He had been looking for centuries. As far as dead ghosts intact I am sure there are ways to kill the light within while not destroying the ghost shell. The Hive extracted the light from living guardians after all.

That's what happens when we enter a Darkness Zone, as when we die in the Darkness Zone, we die permanently, but it's a game so we re-spawn anyway.

Darkness zones and certain evil characters (I'm looking' at you Dredgen Yor) can drain the light out of guardians making it impossible for us to be revived ever again.

Dredgen Yor, look him up, you’ll see what he does. He killed several Guardians with killing their Ghosts.
