You need to complete the milestone(s) for playing 2 quickplay & 2 competitive matches. This is character specific.

Hmmm... I was able to play Mayhem a while back for I believe one of the Faction Rally ornaments, but a friend tells me it was because at the time Mayhem wasn't yet a permanent part of the regular Crucible rotation and was open to all. I tried Competitive to unlock the Milestone, but it was so toxic in there I gave up. Punks messaging me to get out, you suck, stop playing Destiny, etc. This is why I don't like to play PvP. Luckily my friend was able to drag me into Mayhem with him so I could complete the SoH super kills.

Yes. Before, when it was a “special” event, it was open to all. Now that it’s part of the weekly rotation, it must be unlocked to be accessed as fireteam lead.
