Detailed description of crash when loading into tower for Bungie's benefit.
Seeing as Bungie are looking into the tower crash, here's a detailed description of how it happens to me all in one place, for Bungie's benefit.
When I choose to go to a social space (Tower or Farm), it will proceed to the "flying over planet surface"/"flying through space" loading screen as normal, then one of the following will happen:
1) Occasionally I will arrive at the social space as normal.
2) Several seconds later Destiny 2 will crash and take me to the blue screen with a CE-34878-0 error with the option to report the crash to Sony.
3) I will be stuck in that loading screen forever no matter how long I wait. If I go into the inventory/menus at this point the spinny loading hexagon thing will be in the middle of every screen, the model of my character will be missing and nearly every inventory slot and menu button will be blank. A few bits will still show, like the options in the subtitles menu and the volume icons.
If I'm in a fireteam, it seems like my other fireteam members will get stuck on a black screen after having 'arrived' at the social space until I close the app or crash.
If I try to log out or change character (using the blank buttons) the screen will darken slightly and then freeze entirely, BUT the sound of my ship arriving will play and the Tower or Farm music will start playing, though no sounds of social space NPCs. About 10 seconds later, Destiny 2 will crash to the blue screen with a CE-34878-0 error, as before.
It seems to only afflict my characters once they have unlocked the Tower. Before that point, they could reach the Farm 100% of the time, but after each character unlocked the Tower, they started crashing when trying to reach either the Tower or the Farm.
I am on a standard PS4 with its original 500GB HD. I've tried rebuilding the PS4 database, clearing the PS4's cache, freeing up a load of HD space, toggling HDCP, toggling controller mode between Bluetooth and USB, not being in a menu while loading, being in a menu while loading, inspecting an item while loading, being in the PS4 home menu while loading and fiddling with various in-game options but none seem to have affected it.
If I reduce the number of items on my character to at most 6 or 7 per slot, and/or reduce the number of items in my vault to below 175 or so, it SEEMS to make it more likely I will get to a social space successfully, but it could just be coincidence.

This problem has not occurred again since the most recent patch.

As of today the Tower has also been broken in a new way as well as the ways I already mentioned. I seemed to load into the Tower but then was stuck forever on a black screen with NPCs including Tess and Banshee saying things.

Reduce Vault item seem help. I delete 60 item in Vault. With no crush last night.

thank you, an excellent description! I have the exactly same issue(s). one of your 1), 2) or 3)... or if I'm extremely lucky a successful landing at the tower. also, reducing the number of items in vault seem to have a positive effect on tower landing success. (I'm on PS4Pro, so it seem that both models are effected by this annoying bug)

It being a time of day when a lot of players are around SEEMS to make it more likely to get stuck/crash. Also, I can get to any other locations, strikes, crucible etc just fine.
