All of those bug me, but personally, mine is an off shoot of the shotgun problem. Basically, and most practically, user error, but my biggest rage is when -blam!-ing shoot someone in the -blam!-ing face with a -blam!-ing shotgun and the -blam!-ing -blam!-er doesn't even -blam!-ing die. The wording is the exact wording I rage in, but I'm in no way raging now. I just wanted an example for others to compare to.
