Hey there. Try going to Brother Vance then going to the Forge in the Lighthouse to see if you can turn it in.

If you equip Telesto then go to the Lighthouse again, walk over to the forge and use it as you would for one of the Prophecy weapons and you'll receive the masterwork for Telesto (even though you may already have it).
