Is it guaranteed, because I ran it for the first time a week ago. Have all wishes completed but never got one.

Edited by koko: 9/26/2020 8:10:30 AMYes. The Ethereal key is guaranteed to be dropped upon your first enter of the 1st wish on the wish wall, and only the 1st enter. The only other way to get ethereal keys is to kill riven, which you can get once per character per week.
