I’m on pc and keep having an issue where my charged melee bind doesn’t work until I use my uncharged melee bind. I noticed that it tends to happen when I pick up something like a tangle or those treasure you pick up, then I use my praxic blade right after for a minute and that’s when it happens. It didn’t do it when I tried it with a normal sword so I think it’s just with the praxic blade, not sure if anything else causes it.
