Probably already been found but.. The Refinery has one. Patrol from Plaguelands, head to Refinery, it's where the worshipping thrall used to sit that would summon the Crota wizard. That Hive ship thing it'd be crouched in front of, it's door is open and there's a SIVA fragment in there as you turn to face out the door.
