They need to do the following things:
No matches start without being full. Especially during Iron Banner games begin fresh without 6v6.
Give every player a 60 second period upon first spawning in to decide if this game is for them or not. During this time they can leave without penalty.
If they leave after this they are restricted for 20 minutes.

So if I'm kicked out by one of Bungie's numerous animal themed server errors or DDOS'd by cheating scrubs. I also get a 20 minute ban. Or if the entire opposition team start to redbar or lag switch, I've got to stay in the game and take the kicking. Seems unfair to me. I don't quit, though I understand guys who when faced against cheaters do. I will report redbar and laggers but I will stick it out to the end. Punishing guys kicked out of the game for no fault of their own or guys being pounded by redbar gods is not a good solution. Reward players for not quitting. Give them a heroes buff that increases rep gains or loot after a certain number of consecutive games without quitting.

If this was to be implemented it should initially be deployed so that using your ghost to leave is how you get the ban. Then Bungie would have to review internet activity for quitters who just close the app, pull the cord etc and retroactively hand out a much more severe punishment.
