Cortana hitchhikes on the Didact's combat suit and travels through the tear with him, but leaves one fragment of herself within John's suit that remains dormant. John, after the events of Halo 4, takes some much needed R&R aboard the Infinity. While he is sleeping, Cortana's fragment opens a tear into the universe her original is currently in, and brings John through the tear. John then creates memories of Cortana being lost, rather then being dead, and embarks to find her. Edit: I know it's only two games, but I felt I was on to something.
