Last time I noticed anything, I could still pick them up on radar and see an outline pretty well, whilst wearing said helm.

Not sure but the translucent blue shit does a decent job of highlighting them regardless of what helm you wear. Think of it as in intrinsic perk for all helms across all classes.

Invisible nightstalkers highlight invisible nightstalkers.

Only if they're low health, or if their super is ready

Only if their health is low. I have damaged someone and they dodged and went invisible and yet they were still highlighted by the helmet and I killed them.

It just highlights enemies that are low health.

If you have removed the hunter's shields, then it will highlight them in red.
