What does it mean when the gear isn't spinning, but hasn't disappeared like the rest?

That's a bug. If the gear is visible, it's meant to be spinning. You may spin your phone as a temporary workaround.

Just a heads up: This has been fixed. You need not manually spin your phone any longer.

Does it let you know if there are no videos or will the gear continue to spin?

The gear should disappear when it finishes checking that activity or player.
