It should be in the case. If you are looking to redeem go to the Xbox market place or PlayStation store and you should find redeem code around there

I ordered my copy from amazon and it came with nothing but the disk.

There are no codes in the case It only came with the disk and that's it

New or at least I hope so, Amazon doesn't specify if it's new or used but I would assume they ship new games

I've used Amazon before, it's always specified whether or not something is new or used. Unfortunately, if it was used there's not much Bungie can do, if there wasn't a code in the case then it's likely that somebody has already used it. The only thing you're able to do is speak with whoever the game was purchased from.
