Was doing the Saber strike and the warsat landed on me but didn't kill me it just pushed me under the map. It was weird because I could still help cap the warsat tho when I killed myself I was stuck down there.

Was trying to get from Hellmouth to Anchor of Light on the moon, by way of Archer's Line (I was hunting for a specific Vanguard mission). Came in to the area full speed on my sparrow, paying no attention to the public event I thought was already underway, and BAM... direct hit. (Also? Ruined my "Get blah blah blah points without dying" bounty).
