In the adventure for the mindbender once you defeat him as the ascendant realm is breaking there are platforms behind the portal that lead up to a ball that you have to pick up and leave the portal with once you leave there will be a locked door to the right that you use the ball to open the door which will reveal a chest with the emblem in it.
